For Microgrant Recipients:
For reimbursements on purchases you’ve already made, please fill out a Check Request form.
Print out the form and mail it, along with original copies of receipts, to Marisa Parham at Johnson Chapel 108, Amherst College.
Make sure you also submit a W-9, available at this link.
For Student Fellows:
Print out the forms and mail them, along with original copies of receipts, to Marisa Parham at Johnson Chapel 108, Amherst College. If the original receipts are digital, you may email the forms and receipts to firstname.lastname@example.org with the subject title “DH fellow reimbursement request.”
If you’re opting for the digital method you can fill in the forms directly using Adobe Acrobat or Preview (OS X).
